How do I get started with your services?

All therapy services are considered medically necessary, and require a prescription or referral from a physician. If you are a referral coordinator of doctor’s office, please fax the patient’s referral information, including physician’s prescription and all pertinent health information to: (713) 324-7752.

If you are a private pay, or commercial insurance client, we recommend that you contact us at (832) 871-2105 or email: info@custom-therapy.com for more information, and to discuss payment options. We begin with a phone consultation to understand your child’s needs. Then, if needed, once a referral is received from your child’s doctor, we conduct a therapy evaluation or social skills assessment to identify goals for skilled interventions. If your child needs therapy, we develop a personalized therapeutic plan of care (in coordination with the physician) designed to meet your child’s needs. Once approved for therapy visits, your child will receive individualized services from one of our highly qualified therapists on staff.

How much do the services cost and do you accept insurance?

We accept several types of insurance plans, including commercial, Medicaid, and private pay for services. We are in network with several insurances plans, so please contact our office for a complete list of insurances we currently accept. Our dedicated office staff will review your benefits, and if there are any co-pays or balances, we will work to ensure the payment options are agreeable and flexible for you.

What qualifications do your therapists have?

All of our therapists have at minimum, degrees from four-year colleges, and additional training in many specialties; but most have Master’s degrees, and our Physical Therapists have obtained Doctorate Degrees in Physical Therapy (DPT). Additionally, our therapists are state and nationally certified in their respective fields.
